Request for Accommodations under the ADA and the ADA Amendments Act of 2008

It is the policy of The University of Texas Health Science Center San Antonio to comply with the provisions of the Americans with Disabilities Act (ADA), as amended, and Section 504 of the Rehabilitation Act of 1973 (Section 504), as amended. Title II of the ADA provides that “no qualified individual with a disability shall, by reason of such disability, be excluded from participation in or be denied the benefits of the services, programs, or activities of a public entity, or be subjected to discrimination by any such entity.” Section 504 prohibits the exclusion or denial of individuals with disabilities an equal opportunity to receive program benefits and services.

A qualified student, fellow, or resident (student) with a disability requesting an academic accommodation must submit a Student/Resident Request for Accommodation, form ADA-100, to the Office of Educational Support Services with appropriate healthcare provider documentation, if applicable. Upon receipt of a complete request and supporting documentation, an interactive meeting may be necessary. An interactive meeting is a collaborative process where the accommodation request is discussed with the student, OESS, and relevant program representatives to determine necessary, effective, and reasonable accommodation. Students are advised to initiate the accommodation process as early as possible. Incoming or transfer students are advised to start the process as soon as they have been admitted. At any point during the student’s tenure, the student may request additional accommodations or reevaluation of accommodations.

All information provided as part of the accommodation process, including records related to the interactive process, will be maintained in accordance with applicable confidentiality and record retention requirements. Additionally, first aid and safety personnel may be informed, when appropriate, if the disability might require emergency treatment, or if specific procedures are needed in the case of fire or other evacuation. Government officials investigating compliance with the ADA may also be provided relevant information as requested.
